Based on records review 4 out of 12 children enrolled in the facility did not have any required children's documents which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated he will provide proof of all missing children's documents to the Department by 12/2/24.

Based on LPA's review of facility records and interview with licensee, the licensee did not comply with the section cited above as 8 of 12 children enrolled did not have immunization records on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will complete all immunization records for the 8 children in care and submit proof to LPA by POC date of 12/2/24

Based on records review and interview, the licensee did not have children's roster when requested which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee told LPA that he will complete the LIC9040 Facility Roster and will email proof to the LPA by 12/2/24.

Based on LPA's observations during facility inspection, stairs were not fenced or barricaded. This poses a potential health and safety risk to children in care..

Resolution: Licensee placed the safety gate at the bottom of the stairs during today's inspection. This deficiency has been cleared.

There are 4 electrical outlets missing covers in living room, which poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee told LPA that he will purchase and install outlet covers and email proof to the LPA by 12/2/24.

Based on record review, the licensee did not comply with the section cited above as his mandated reporter certificate expired 1/19/22, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated he would renew his mandated reporter training and send updated certificate to LPA via e-mail by 12/2/24

Based on record review, the licensee did not comply with the section cited above as today helper Shahla Motazedi online mandated child abuse training certificate had expired which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that he will have his helper take the online mandated child abuse training course and will submit a copy of the certificate to Analyst by 11/29/2021. Licensee shall submit a written plan of correction on what he will do to ensure that the facility does not violate this regulation in the future.

Based on observation, the licensee did not comply with the section cited above which a safety gate is required to be placed at the bottom of the stairs which poses potential health, safety or personal rights risk to persons in care.

Resolution: Licensee located the safety gate from the bedroom closet during the inspection and placed the gate at the bottom of the stairs. Licensee shall submit a written plan of correction on what he will do to ensure that the facility does not violate this regulation in the future.
