Resolution: Licensee's husband arrived and replaced the batteries but it still didn't work so he went to purchase a new CO detector during the visit. deficiency was corrected.

Resolution: Licensee stated she will obtain copies of the children's immunization records and submit to LPA by POC due date of 10/14/22 or provide written plan to obtain copies of enrolled children's immunization records and the records for newly enrolled children going forward. Immunization record for 1 child found prior to exit.
Based on interview and record review, the licensee did not comply with the section cited above as her CPR and First Aid certifications were not renewed upon expiration in 6/2021 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will call to register for CPR/ FA certification tomorrow and provide LPA with confirmation of enrollment. POC due date will be extended if necessary upon request by licensee, but no longer than 60 days from today.
Based on observation, interview, and record review, the licensee did not comply with the section cited above as the posted drill log had no entries and licensee acknowledged drills had not been conducted which posesa potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will conduct and document a disaster drill within the next 2 weeks, when a child is in attendance. If no children attend within the next 2 weeks licesnee stated she will provide a written plan of action detailing he runderstanding of the regulation and how she will comply going forward.
Based on observation and interview, the licensee did not comply with the section cited above, her 24 year old son had not been fingerprinted after his 18th birthday which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated her son will get live scan the next day and submit a copy of the receipt and LIC 508 to LPA by POC due date of 9/23/22.
