Type B citation from complaint investigation - potential risk if not corrected
Based on LPA observation and records review licensee did not ensure medication was properly handled and stored as medication for C1 was not in its original container with prescription and the medication for C3 was expired. which poses a potential risk to the health and safety to children in care. This is an amended version of the original report dated 8/25/2021.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirements was not met as evidenced by: Based on interviews conducted and records review, the licensee did not ensure a safe environment as high levels of lead were detected in a classroom and restroom which posed a potential health and safety risk to children in care.
Based on interview and record review, the licensee did not comply with the section cited above in no staff member had a vaild CPR/FA certification on file for review which poses a potential health, safety or personal rights risk to persons in care.
Based on records review and owner's admission owner did not ensure S3 had a criminal record clearance or exemption prior to working, residing, or volunteering in the licensed facility as required, which poses an immediate Health and Safety risk to the children in care.
LPA observation and owner's admision owner did not ensure the condtions and limitations as sepcified on the license were observed as there were 24 children in attendance and the facility's capacilty is for 20 which poses an immediate Health and Safety risk to the children in care.
Based on owner’s admission owner did not ensure the center has a qualified director since June 7, 2021.

(f) At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to Health and Safety Code Section 1596.866 shall be present when children are at the child care center or offsite for center activities.
