Based on interview's and record/document reviews, the facility did not comply with all terms and conditions stated in the admission agreement. This poses a potential health and safety risk to children in care.
Based on water testing results and interviews, facility tested over the Lead Exceedance level at 3 sinks and 2 drinking fountains (faucets) measuring at 8.120ppb, 6.150 ppb, 11300.000ppb, 7.130ppb, 787,000ppb. This poses an immediate health, safety or personal rights risk to children in care.
(i)(2) Personnel Requirements: Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not met as evidence by: Based on observation and record review. Staff #1 is clear but not associated to the preschool program. This posses an potential risk to the health and safety of the children in care.
