Based on observation, the licensee did not comply with the section cited above by having multiple debris items including small pieces of garbage and very small toys which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee stated she will clean up the debris and small items inside and outside the home that pose as choking hazards to young children in care and keep all garbage seperated from children's play areas. The Licensee stated that she would complete the personal rights training on the CCLD child care videos website sent to the Licensee via email by 7/25/2025.

Based on observation, the licensee did not comply with the section cited above by blocking the second fire exit with objects and also by having a pipe/pole coming out of the ground at an angle that can be fallen on which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The LIcensee stated she will clean up the fire exit to be free from all items blocking the clearance area and Licensee stated she will either barracade or remove the area where the pole is sticking out from the ground. The Licensee stated that she would complete the personal rights training on the CCLD child care videos website sent to the Licensee via email by 7/25/2025.

Resolution: The Licensee stated that she will sign up for CPR/FIrst Aid for her and her mother who is her assistant no later than next week. The Licensee stated tht whe will let LPA Williams know by email at: shannan.williams@dss.ca.gov 7/17/2025 which date they are signed up for. The Licensee stated that she would complete the health and safety training on the CCLD child care videos website sent to the Licensee via email by 7/25/2025.

The Licensee stated that she would complete the health and safety training on the CCLD child care videos website sent to the Licensee via email by 7/25/2025.

Resolution: The Licensee stated that she would have one out of one child's authorized representative fill out the two forms. The Licensee stated the other child was a emergency drop off and will not be enrolled into the facility, but if they do enroll him that she will get the forms filled out.
