Based on staff interview and parent interviews, staff #1 did not treat child with dignity. This poses an immediate health and safety risk to children in care.

Reporting Requirements: The items below shall be reported to the Department within 10 working days following their occurrence: Any changes in the plan of operation that affect services to children. This requirement is not met as the children were supposed to follow the staggered napping scheduled that the facility representative had submitted to the department by having children nap in classroom #1 due to the fire clearance conditions. A licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidence by: On 05/12/2023, five out of seven staff members online mandated child abuse certificate had either expired or was not available for review. Personnel Requirements Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This requirement is not met as evidence by: On 05/12/2023, two out of seven staff records did not have the LIC503 health screening report in the staffs files. Staff C.L.and K.S. are missing the LIC503. This poses a potential health and safety risk to children in care.
