Based on record review, staff Samantha Seabrooks' fingerprint clearance is not associated to the facility. Failure to associate staff poses a potential risk to the health, safety or personal rights of children in care.

Based on the self-reported incident, staff in charge of the group failed to provide appropriate supervision wherein 4 children were engaged in inappropriate activities for 3 days. Children went under a blanket and looked at each other's genitals. Staff's failure to provide appropriate supervision poses an immediate risk to the health, safety or personal rights of children in care.

Facility failed to provide adequate supervision to children who is currently participating in the program when school was dismissed at 12:15PM and there were no staff to provide supervision.

Based on LPA's interview with staff, it was determined that staff failed to provide adequate supervision when children were dismissed from school at 12:15PM. Staff did not arrived on site until 1:05PM.

Based on staff interview, staff failed to report to the department an incident involving staff's failure to provide adequate supervision when on 3/13/24, there were no staff available to supervise 18 children when they were dismissed from the elementary school.

Based on record review, the licensee did not comply with the section cited above. LPA was unable to verify site director's completion of Administration class. Facility's failure to maintain director's proof of Administration course completion poses a potential health, safety or personal rights risk to persons in care.
