Based on observation, the licensee did not comply with the section cited above. The drawer in the kitchen (next to refrigerator) was observed accessible to children. The latch broke and did not work. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Mrs. Jules removed the sharps and butcher knife and made them inaccessible to children. Mrs. Jules stated that she will fix the latch no later than 9/7/2023. Mrs. Jules will send a photo to show that the latch was fixed no later than 9/7/2023.

Based on observation, the licensee did not comply with the section cited above. The wall heater was observed accessible to children. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Jules stated that she will install a barricade to make the wall heater inaccessible to children no later than 9/13/2023.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Jules failed to conduct a fire drill with the children. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Jules stated that she will conduct a fire drill no later than 9/13/23 with the children. She will submit a copy of her fire drill no later than 9/13/23.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee's sister Bertha is her helper. Licensee failed to obtain the required immunization for Bertha. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Jules will obtain Bertha's immunization record and submit copy to the department no later than 9/13/23.

Based on interview & record review, the licensee did not comply with the section cited above. Several children were missing form LIC 700. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Mrs. Jules a packet of children's forms. She will obtain completed forms from the parents and submit to the department no later than 9/13/2023.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee failed to maintain the required form LIC 282. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain completed form from the parents and submit copies to the department no later than 9/13/2023.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Jules did not maintain a sleeping log that documented the 15-minute check during naps. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided the safe sleep regulation packet to Mrs. Jules. She stated that she will start logging the 15-minute checks during nap.
