JOFEY, SAHRA FAMILY CHILD CARE

Based on LPAs Siao and Poindexter observation, the licensee did not comply with the section cited above as there were two adults that have prominate presense and were sleeping at the facility did not have the required background clearances. Licensee's son Ali Mohamed Jama truck driver stated that he only stays for about a month each time and he does not have the required background clearances. Licensee's nephew Salim Mohamed Abdulahi who was sleeping at the home and has a letter with the faciilty's address that was observed by LPA Siao does not have the required background clearances. This poses an immediate health, safety or personal rights risk to persons in care. Civil peanlty of $1000 is assessed today.

Resolution: Licensee that that her older son and her nephew has other places that they can stay rather than the facility.

Facility does not have any records available for review, the licensee did not comply with the section cited above in 4 out of 4 children from two that are from two families does not have the LIC995B Regarding Removal/Exclusion for licensee's son Omar Jama. This poses an immediate health, safety or personal rights risk to persons in care. Civil penalty of $200 for two families will be issue today.

Resolution: Licensee's daughter stated that she will ensure to have the families sign the LIC995B regarding removal/exclusion form for (Omar Jama). She will submit the forms to LPA Siao no later than 06/22/23.

Based on LPAs Siao and Poindexter observation, the licensee did not comply with the section cited above as there were three rolls of area rug at the end of the hallway, laundry room and the storage room that area accessible to the children. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will put a gate at the hallway to prevent the children access to the off limit rooms and hallway to ensure children's safety.

Based on LPA Siao's observation, the licensee did not comply with the section cited above as there was a sharp knive in the kitchen drawer that is not latch and accessible to children. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the sharp knive in the kitchen drawer during the inspection.

Based on records review, the licensee did not comply with the section cited above as the last drill documented on the fire drill log that is posted on the wall indicate that they had a drill in the year of 2020. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's daughter Amina Jama stated that she will conduct a fire drill with the children in care and she will email the drill log to LPA Siao no later than 06/30/2023.

Based on record review, the licensee did not comply with the section cited above as helpers Amina Jama and Hoden Jama stated that they have not taken the online mandated child abuse training. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licencee's daughter Amina Jama stated that her and her sister Hodan Jama will complete the online mandated child abuse training and will submit it to LPA Siao no later than 07/14/2023.

Based on record review, the licensee did not comply with the section cited above as licensee and her daughter/helper Hodan Jama's measles and pertusis/TDAP immunization records are not available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Helper Amina Jama stated that her mom Sahra Jofey and sister Hodan Jama will obtain their measle and tdap immunization from their doctor and submit the documents to LPA Siao by 07/14/2023.

Based on record review, the licensee did not comply with the section cited above as the four children's immunization records are not available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Helper Amina Jama stated that she will obtained the children's immunization records and will transfer it to LPA no later than 07/14/23.
