Based on record review, the licensee did not comply with the section cited above in not providing the department with documentation of the safe sleep log, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee provided the department with a completed safe sleep log at todays inspection. Licensee will provide the department a written statement of her understanding of this regulation by email by 11/22/24.
Based on observation, the licensee did not comply with the section cited above in having cleaning compounds in the storage room that is used as a passage way to the backyard accessible to children which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the cleaning compounds and put them in an inaccessible area at the time of this inspection.
Based on record review, the licensee did not comply with the section cited above in not providing current Mandated Reporter Certificates for Licensee, Staff #1, and Staff #2, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee stated she will send the department by email copies of current mandated reporter certificates for Staff#1 and Staff #2 by 12/13/24 with a written statement of her understanding of this regulation.
Based on record review, the licensee did not comply with the section cited above in not providing the department a negative TB test result for Staff #2, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send the department a copy of a negative test result for Staff #2 by 12/13/24 by email and a written statement of her understanding of this regulation.
Based on record review, the licensee did not comply with the section cited above in not providng the department a copy of the LIC9052 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will provide the department a copy of a signed LIC9052 for Staff #2 by email by 12/13/24.
Based on record review, the licensee did not comply with the section cited above in not providing proof of immunizations for Child #1, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send the department a copy of Child #1's immunizations by email by 12/13/24.
Based on observation and interview, the licensee did not comply with the section cited above and has not conducted fire and disaster drills every six months as required which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will conduct a fire drill the week of 11/7/22 and submit a copy of the completed drill log to analyst by 11/14/22 to complete the correction.
Based on interview and record review, the licensee did not comply with the section cited above as she is not documenting the four (4) infants she has in care's sleeping status every 15 minutes which poses/posed a potential health, safety or personal rights risk to the child in care.
Resolution: Licensee was provided with a sample Safe Sleep log and stated she will begin documenting infants sleeping status every 15 minutes. Licensee will send a copy of the completed log for the dates of 11/7/22 - 11/11/22 to analyst by 11/14/22.
Based on observation, review and Licensee's statement, Licensee's Mandated Reporter Training has expired and two (2) out of three (3) helpers, S1 and S3 have not completed the training, the licensee did not comply with the section cited above which poses as a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will ensure that all three (3) persons, licensee, S1 and S3 will complete AB-1207 Mandated Reporter training and send certificates to the San Diego Child Care Regional Office (SDCCRO) as proof of correction by the due date of 12/04/2022.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that three (3) out of (3) helpers, S1, S2, S3 did not have the required immunizations for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure all persons receive the required immunizations or show proof of documentation of the required immunizations and send proof to the San Diego Regional Office by the due date 11/14/22.
Based on record review, the licensee did not comply with the section cited above in that (8) out of (8) children's files were incomplete, missing signatures and information on emergency cards and other required forms which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee the LIC311D which lists required forms for children's files. Licensee stated she will provide the SDRO with copies of the eight (8) children's completed files to LPA by 11/14/22.
Based on observation and the Licensee's statement that there is no LIC 9227 form for infant, the licensee did not comply with the section cited above in that two (2) out of four (2) infants lacks a completed LIC 9227 Individual Sleeping Plan on file, which poses as a potential health, safety or personal rights risk to children in care.
Resolution: LPA provided Licensee with a blank LIC 9227 form and PIN 20-24-CCP "Safe Sleep Regulations". The Licensee stated she will submit a copy of the completed LIC 9227 for the infants in care for the plan of correction to the San Diego Regional Office by 11/14/2022.
