Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Ismail failed to conduct a fire drill with the day care children. Mrs. Ismail's last fire drill was in June 2022. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Ismail stated that she will conduct a fire drill when children arrive today. She will submit a copy of her drill log to the department no later than 5/26/2023.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee's helper Ayan Bashir did not have the required immunization on file - Pertussis, Measles & Influenza. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Ismail stated that she will obtain a copy of Ayan Bashir's immunization record and submit a copy to the department no later than 5/26/2023.
