Based on interviews with the licensee and adult resident, and record review, the licensee did not comply with the section cited above in one out of one adult residents were fingerprinted in June 2023; however, the fingerprints were not associated due to the incorrect facility number which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: During the inspection, Licensee provided the adult reside with the LIC 9163-REQUEST FOR LIVE SCAN SERVICE - COMMUNITY CARE LICENSING form. The adult resident left the facility to have the fingerprint process completed by Livescan. The licensee stated she will provide the LPA with the completed LIC 9163. The licensee stated the adult resident will not be present while children are in care until associated to the facility.
Based on record review and interview with the Licensee, the licensee did not comply with the section cited above in five out of five persons did not renew or complete the mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated she will provide proof of Mandated Reporter training certifications for employees/licensee no later than 09/08/2025. The licensee stated she understands all staff must complete the training prior to working in the facility, renew every two years and place a copy in the employee files. www.mandatedreporterca.com
Based on record review and interview with the licensee, the licensee did not comply with the section cited above in four out of four employees did not complete the LIC 9052 prior to working at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated she will have the employees complete the LIC 9052 and place a copy in their file. The licensee stated she understands all current and new employees must complete the LIC 9052 and place in the file for the Department to review.
Based on LPA's observation and interviews with children in care, the licensee did not comply with the section cited above that the facility’s designated child care room, living room and dining area were not maintained in a manner that provided comfortable accommodation for the children in care. The daycare room was excessively hot, requiring the LPA to close the door to prevent heat from flowing into the remainder of the home. The LPA also requested that the fan in the living room window be turned on to assist with circulation which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated she will ensure the daycare areas are maintained at a safe and comfortable temperature at all times when children are in care. The licensee stated she will utilize fans and portable air conditioning until the solar panels are set up properly. The licensee stated she will monitor the rooms throughout the day. The licensee will provide the LPA proof of photos of ventilations devices in place and a written statement of ongoing monitoring no later than 09/03/2025.
Based on the licensee admitted the staff check and monitor the infants under two years however, the staff do not documented], the licensee did not comply with the section cited above in three out of three infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated as of today the staff will start documenting the infants two years and under. The licensee will send the LPA a sleeping log for the three infants no later than 07/18/2023.
Based on the licensee admittance, the licensee did not comply with the section cited above in one out of one staff personnel reords were not available at the facility for the dept. to review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated she will provide the staff members personnel record to the LPA no later than 07/18/2023.
