Based on observation, the licensee did not comply with the section cited above as analyst found an aerosol Raid ant and roach killer can in an unlatch and unlocked closet in the hallway which poses an immediatel health, safety or personal rights risk to children in care.

Resolution: Licensee removed the Raid can during today's visit and moved it to a different closet and secured it with a lock.

Based on observation and Licensee's statement that she has not taken the Mandated Reporter Training, the licensee did not comply with the section cited above in that she has not completed the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will complete this online course and provide LPA with a copy of her training completion certificate no later than 05/25/23.
