Based on interview and record review, the licensee did not comply with the section cited above. Licensee did not have proof of fire drill conducted. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of fire drill conducted to LPA via email to Annette.Sutherland@dss.ca.gov by 4/26/24.

Based upon observation, file review and conversation with Licensee, no documentation was not on file for Infants #1 - 4. This is a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide safe sleep logs for infants in care and send proof to LPA by 4/26/24 to Annette. Sutherland@dss.ca.gov Sleep log documentation shall include date, infants name and time of each 15 minute check and observation .

Resolution: Licensee will submit proof of complete children's files for the 8 children in care by 4/26/24 to LPA Annette Sutherland@dss.ca.gov

Based on interview and record review, the licensee did not comply with the section cited above. Licensee was unable to provide a roster for children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a complete roster LIC 9040 to LPA by 4/26/24 to LPA Annette.Sutherland@dss.ca.gov

Based on observation, the licensee did not comply with the section cited above. LPA observed 2 infants in sleeping in play pens. Observed were blankets, pillows, stuff animals and bottles in the play pen. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: CORRECTED TODAY. Mrs. Hirsi removed the loose articles from the play pen today.

Based on observation, the licensee did not comply with the section cited above. During a tour of the facility, LPA observed several bottles of cleaning compounds (under the kitchen and bathroom cabinets), knives in the kitchen drawer, accessible to children in care. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: CORRECTED ON THE SPOT. Mrs. Hirsi removed the cleaning compounds and knives and made them inaccessible to children. She will remind her room mate to ensure that these items are kept inaccessible to children.

Based on observation and record review)], the licensee did not comply with the section cited above. Upon arrival, Lucky Egal was observed supervising 2 infants in care. She stated that Mrs. Hirsi stepped out to the grocery. She stated that she was a friend and does not work for Mrs. Hirsi. Mrs. Egal does not posses a Pediatric CPR/First aid certificate. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Hirsi stated that she will have Lucky Egal complete a Pediatric CPR and First Aid class no later than 3/24/2023. She will submit a copy of her certificate via email to Nancy.Diaz@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above. The licensee and Mrs. Egal were not able to provide this analyst a copy of the required immunization to show that they have been immunized against Pertussis, Measles and Influenza. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Hirsi shall submit a copy of her and Mrs. Egal's immunization to the department no later than 3/24/2023.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Hirsi did not have form LIC 9227 for one infant in care (under 12 months). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Hirsi shall provide form LIC 9227 for infant's parent to fill out. She will submit a copy to the department no later than 3/24/2023.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Hirsi did not maintain a log for the 15-minute nap check. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Mrs. Hirsi a sample nap log form. She stated that she will check on the infants when they nap and log the 15-minute checks starting today. A copy of nap log shall be submitted to the department no later than 3/17/2023.
