HAYDER, SAHRA FAMILY CHILD CARE

Based on analyst observation, the licensee did not comply with the section cited above as her smoke detctor was non-operational when inspected by analyst during visit which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states she will purchase a new smoke detector and send analyst a photo of the purchase and receipt along with a video of smoke detector being tested by 09/26/25 to complete the correction.

Based on record review, the Licensee did not comply with the section cited above in that Licensee’s Mandated Reporter Training was unavailable for review during inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will ensure to complete AB-1207 Mandated Reporter training and send certificate to the San Diego Child Care Regional Office (SDCCRO) as proof of correction by the due date of 10/10/25.

Based on record review, the licensee did not comply with the section cited above in (2) out of (3) children's files were missing required documents which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee the LIC311D which lists required forms for children's files. Licensee stated she will provide the two children's completed files to LPA by 09/26/25.

Based on analyst record review, the licensee did not comply with the section cited above as it was determined that she is not maintaining a facility roster which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Analyst provided licensee with a blank facility roster during today's visit. Licensee states that she will fill out the roster listing all the children in care and their corresponding required personal information and then submit a copy of the completed roster to analyst by 09/26/25 to complete the correction.

Based on observation, record reviews and Licensee's statement that immunization records for Child 1 -4 were not in the facility, the licensee did not comply with the section cited above in that four (4) non school aged children did not have immunization records in the facility, which poses as a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states they will provide LPA with copies of the immunization records of Child 1 -4 (See LIC 811 Confidential Names) no later than 05/31/2023.
