Based on record review, the licensee did not comply with the section cited above. Mrs. Hade has not conducted a fire drill since 2019. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Hade stated that she will conduct a fire drill with the children today, 3/9/23 and will submit a copy of the fire drill to the department no later than 3/16/23.

Based on record review and Mrs. Hade's statement, the licensee did not comply with the section cited above. Her son, Abdifatah Muse Hirsi arrived from Africa in December 2022. Mr. Hirsi's fingerprint clearance is not associated to this facility. His fingerprint is associated to the Child Care Center owned by Mrs. Hade. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Mrs. Hade form LIC 9182. She must complete the form and submit to the department no later than 3/16/2023.

Based on record review, the licensee did not comply with the section cited above. Helper Ayan Hirsi did not have a copy of immunization record for the department's review. She stated that she does not have immunization for Measles, Pertussis and Influenza. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Ayan Hirsi stated that she will obtain the required immunization and provide the department a copy of her immunization to show that she was immunized against Pertussis, Measles and Influenza no later than 3/16/2023.
