Resolution: Licensee will obtain immunization record and place it in the file of C1. Moving forward Licensee will ensure that all immunizaiton records will be maintained in each child's file when applicable.

Resolution: Licensee will will obtained signed LIC 995A for all children in care. Moving forward licensee wiill ensure the LIC 995A is complete prior to a child being accepted into the facility.
