Resolution: Moving forward licensee will maintain a safe sleep log for all infants in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 3 children's files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete the LIC 9227 with the parent of C2. LIcensee will obtain a completed LIC 9227 for all children 0-12 months prior to enrollment in the facility.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out 3 children's files were missing the signed LIC 995A which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtained a signed LIC 995A for C1 and C2. Moving forward licensee will obtain a signed LIC 995A prior to childrens enrollment in the day care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above, CPR/ First Aid expired 4/2021 and was not renewed which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she is in the proess fo registering for the class and has contacted the vendor. She stated she will submit proof of registration by POC due date and submit copy of certification within 30 days of POC due date.
