Based on interview and record review, the licensee did not comply with the section cited above in licensee failed to renew the required Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she will ensure to have her assistant take the required course training this week and will send a copy of her AB1207 Mandated Reporter certificate to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
Based on observation, the licensee did not comply with the section cited above due to the playground structure not being secured to the ground which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she would have the playground structure fixed or remove them by the due date. Provider will have structure fixed or removed for LPA to see by due date.
Based on interview and record review, the licensee did not comply with the section cited above in not conducting an emergency drill which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated she will conduct an emergency drill and send proof of completion to the San Diego Child Care Regional Office (SDCCRO).
Based on interview and record review, the licensee did not comply with the section cited above in not having the required Infant Sleeping Log on infant in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she will ensure to fill out a form daily for every infant under the age of 24 months and will send copies of the week 2/6/25-2/12/25 to the SDCCRO as proof of correction.
Based on observation, the licensee did not comply with the section cited above due to the playground structure not being secured to the ground which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she would have the playground structure secured to the ground or remove them by the due date. Provider will send pictures to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
Based on observation, the licensee did not comply with the section cited above, LPA observed a bottle of lotion and tooth paste on top of the daycare bathroom sink and several personal hygiene items on the open shelves located above the toilet, all accessible to daycare children which poses potential health, safety or personal rights risk to persons in care.
Resolution: Provider removed all items from the bathroom sink and shelves during the inspection. No further plan of correction needed. Deficiency was corrected during inspection.
Based on interview and record review, the licensee did not comply with the section cited above in one of the staff present during the inspection failed to renew the required Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she will ensure to have her assistant take the required course training this week and will send a copy of her AB1207 Mandated Reporter certificate to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
Based on interview and record review, the licensee did not comply with the section cited above in not having the required Individual Infant Sleeping Plan on infants in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she will ensure to fill out a form together with parent for every infant under the age of 12 months and will send copies of the plan to the SDCCRO as proof of correction.
