Based on record review, the licensee did not comply with the section cited above in 1 out of 1 Recording and performing a fire/ eathquake drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will perform a fire and earthqueake drill and send copy of log to LPA Via regualr mail.

Based on observations and record review, the licensee did not comply with the section cited above in 3 out of 3 children sleeps logs are not being conducted which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send current sleep logs to LPA for review via regular mail to RO office.

Based on record review, the licensee did not comply with the section cited above in 1 out of 1 Staff records incomplete which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will comply on having a completed staff record and will take assistance from TSP. LPA will submit referal upon return to RO.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee and all three assistants do not have current mandated repoerter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will ensure that all assitants complete mandated reporter training and will provide proof to LPA no later than no close of business 6/20/22 and will ensure that all care providers complete mandated reporter training every two years hereafter.

Based on interview and record review, the licensee did not comply with the section cited above as she does not have staff files for 3 of 3 assistants or self which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide complete staff files for staff #1, staff #2 and staff #3 and self to LPA no later than close of business 6/20/22.

Based on observation and record review, the licensee did not comply with the section cited above as Licensee does not have a current child roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide current roster of children to LPA no later than close of business 6/20/22.

Based on record review the licensee did not comply with the section cited above as 13 of 13 children do not have complete child files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will ensure that all children in care have complete child files and provide proof to LPA no later than close of busienss 6/20/22.

Based on observation, interview and record review, the licensee did not comply with the section cited above as she currently has in care 13 infants and children with no school aged children present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states will disenroll one child and provide proof to LPA no later than close of business 5/24/22

Based on observation and interview the licensee did not comply with the section cited above as there is no carbon monoxide detector in the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will install carbon dioxide detector and provide LPA with proof no later than close of business 6/20/22

Based on observation and record review the licensee did not comply with the section cited above as adult #1 does not have fingerprint clearances which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will ensure that adult #1 completes livescan, including CACI and provide proof to LPA no later than close of business 5/24/22.
