Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Farah did not have a copy of Mr. Hassan's current Pediatric CPR/First Aid on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Farah will submit a copy of Mr. Hassan's current Pediatric CPR/First Aid to the department no later than 10/19/23.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Farah did not maintain a personnel record for helper Abdirahaman Hassan. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Farah did not maintain personnel record for helper Abdirahman Hassan. LIC 501 was provided today. She will have her helper Ms. Hassan complete the form no later than 10/19/23. She will submit a copy of this form no later than 10/19/23 via email.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Farah did not maintain a signed LIC 9052 for helper Abdirahaman Hassan. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Mrs. Farah form LIC 9052. She will have her helper sign the form and submit a copy to the department no later than 10/19/23.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Farah did not maintain personnel record at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Farah shall submit a copy of completed forms to the department no later than 10/19/23.

Based on observation, the licensee did not comply with the section cited above. LPA observed a part of the fence on the ground with nails sticking out. LPA also observed a bucket with mop, vacuum cleaner and mattress stored in the yard. These were observed accessible in the yard where children play. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Mrs. Farah removed fence material with nails out of the yard. She was also observed removing the bucket with mop and vacuum cleaner out of the yard. She stated she will remove the mattress from the yard no later than end of business day of 10/12/23. She will submit a photo of the yard to show that the mattress was removed.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Farah did not have a copy of helper's Abdirahaman Hassan's current Mandated Reporter certificate. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Farah will submit a copy of Abdirahaman Hassan's current Mandated Reporter Training certificate no later than 10/19/23.
