Based on interview and record review, the licensee did not comply with the section cited above in not having updated current children's roster which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated they will update children's roster and send proof to SDRO.

Based on interview and record review, the licensee did not comply with the section cited above in not conducting an emergency drill since 3/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees stated they will conduct an emergency drill and send proof to SDRO

Based on observation, the licensee did not comply with the section cited above in 3 "off limit" rooms were not made inaccessible to children which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee locked door from inside room, LPA advised licensee to keep doors locked during hours of operation

Based on observation and interview, the licensee did not comply with the section cited above in advsing the LPA that niether licensee were home. When licensee Litha Desauguste was home with children in care this poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensees stated they understood, opened door and allowed LPA to continue inspection of home.

Based on observation and record review, the licensee did not comply with the section cited above in 10 of 13 files werent complete. All children were missing emergency information which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensees stated they will complete all childrens file and submit to SDRO.

Based on observation, the licensee did not comply with the section cited above in licensee did not provide both licensee phone number which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee provided updated LIC 279 with both licensee phone number.

Based on analyst interview and record review, the licensee did not comply with the section cited above as they are not documenting the infant they have in care's sleeping status every 15 minutes as required by regulation which poses a potential health, safety or personal rights risk to the child in care.

Resolution: Licensee was provided with a sample Safe Sleep log and stated they will begin documenting infants sleeping status every 15 minutes. Licensee will send a copy of the completed log and send it to analyst by 04/05/2024.

Based on analyst observation and interview, the licensee did not comply with the section cited above as they have not been documenting the fire and disaster drills every six months as required which poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee was provided with a blank fire/earthquake drill log and states will conduct both drills with the children in care this week and then continue to do so once every six months as required by regulation. Licensee states they will submit a copy of the completed drill log to analyst by 04/05/2024 to complete the correction.
