Based on observation, interview, record review, the licensee did not comply with the section cited above in that the documentation recorded for infants safe sleep did not include position infant was sleeping in or infant's condition which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above in that four (4) infants who could climb out of cribs did not have a mat or cot to sleep on which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above in that six (6) infants bottles and food only contained a name label and not a date which poses/posed a potential health, safety or personal rights risk to persons in care.
