Based on interview and record review, the licensee did not comply with the section cited above. Mrs. De la Cruz failed to maintain vaccination record for helper Aurea Romero - measles, pertussis and influenza. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Dela Cruz will obtain a copy of Mrs. Romero's vaccination record to prove that she has been immunized against Pertussis, Measles and Influenza. Copy of vaccination is to be submitted to the department via email to Nancy.diaz@dss.ca.gov no later than 4/25/2024.
