Based on observation, the licensee did not comply with the section cited above as carbon monoxide detector is not operational which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states will repair/replace carbon monboxide detector and provide proof to LPA no later than close of business 3/30/23.

Based on observation, the licensee did not comply with the section cited above as smoke alarm is not operational which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states will repair/replace smoke alarm and provide proof to LPA no later than close of business 3/30/23.

Based on observation, the licensee did not comply with the section cited above as Licensee entered off limits bedroom with a child following and there were medications and other hazards on the dresser which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee removed child immediately at LPA request and stated will ensure that in the future, off limits areas stay inaccessible to children at all times when children are in care.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee states does not conduct disatser drills every six months and cannot remember which date last year she conducted a disaster drill, as she does not keep records of drills which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will conduct a disaster drill and provide proof to LPA no later than close of business 4/5/23 and will in future ensure that all disaster drill are conducted every 6 months and documented.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee does not keep sleep logs for child 2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide sleep logs for child 2 no later than close of business 4/5/23 and will, in future,ensure that all children under 2 years old have sleep logs documenting 15 minute nap checks for every infant and every nap.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee unsure if has daycare liability insurance as unable to find it and does not have signed LIC282 Affidavit Regarding Liability Insurance for Family Child Care Home which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will provide proof of liability insurance or LIC282 signed by each parent of a child currently in care no later than close of business 4/5/23 and will in future ensure that either a liability insurance policy is available or that all child files have the LIC282 signed by parents of children in care.
