Based on record review, the licensee did not comply with the section cited above in that the staff assistant (S1) did not have a completed Mandated Reporter Training certificate available for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will ensure assistant (S1) complete this online course and provide LPA with a copy of her training completion certificate no later than 02/06/2026.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that assistant (S1's) TB immunization was dated 2010, and not more than one year prior to employment which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will ensure assistant (S1) receives a current negative TB immunization and provide LPA with a copy of the results no later than 02/06/2026.
Based on record review, the licensee did not comply with the section cited above in (4) out of (8) children did not have the form LIC627 for review in children's files which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee the LIC311D which lists required forms for children's files. Licensee stated she will provide the copies of the missing forms from the four (4) children's files to LPA by 1/20/26.
Based on record review, the licensee did not comply with the section cited above in (4) out of (8) children did not have the form LIC282 Liability Insurance for review in children's files which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee the LIC311D which lists required forms for children's files. Licensee stated she will provide the copies of the missing forms from the four (4) children's files to LPA by 1/20/26.
