Based on observation, the licensee did not comply with the section cited above in having accessible cleaning detergent to daycare childre, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the cleaning compound at the time of this inspection. Licensee stated she will submit a written statement of her understanding of this regulatio. License signed the Health and Safety Issues form stating she understands this regulation by email by 5/16/25.
Based on record review, the licensee did not comply with the section cited above in not providing current Mandated Reporter Certificates for both Licensee and Staff Member, Vizcarra, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: License stated she will send the department copies of the Mandated Reporter Certificates for both Licnencee and Staff Vizcarra by email by 5/16/25.
Based on record review, the licensee did not comply with the section cited above in not providing the department with immunizations for Staff Member, Vizcarra, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee statesd she will send the department a copy of immunizaitons for Staff Vizcarra by email by 5/16/25.
Based on observation, the licensee did not comply with the section cited above as LPA osberved Staff Member, Vizcarra was alone with 7 daycare children 4 of whom were infants, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee left with one infant to take home and stated she will disenroll a child from the facility to be withing ratio. Licensee also stated she will make sure she is always within ratio. Licensee stated she will submit a written statement of her understanding of this regulation by 5/16/25.
