Based on observation, interview and record review, the Co-Licensee's and assistants did not comply with the section cited above in that all residents and assistants did not have immunization records on file for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Co-Licensee stated she will submit proof of immunizations for all staff and residents by 8/31/22 and will submit a copy for the plan of correction to the San Diego Regional Office by 8/31/22.
