Based on observation and interview, the licensee did not comply with the section cited above in that the fire extinguisher has expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure the existing fire extinguisher is recharged or replaced and the valid fire extinguisher is available, to be in compliance.
Based on interview and record review, the licensee did not comply with the section cited above in that the licensee's mandated reporter certificate was unavailable for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure she registers and completes the Mandated Training course and submiti the completed certiicate to the SDRO by September 18,2025.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that all three (3) infants present had files that were missing required children's forms which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided licensee with the LIC311D resource form which lists the required children's forms. Licensee stated she will provide the parents with the required forms to fill out and return. Licensee will make copies of the files and submit to the SDRO by 8/29/2025.
