Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out of 5 kids files were missing immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain immunization records for C1 and C2 and place in childs' files. LPA will verify corrections with additional visit.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 2 out of 5 children's file were missing the LIC 995A which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain signed LIC 995A for C1 and C2 and place in child's files. Analyst will verify corrections with additional visit.
Based on observation, interview, and record review the licensee did not comply with the section cited above in 1 out of 1 infant file was missing the LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete LIC 9227 with the parents of C3 and place in child's file. Licensee will verify correction with additional visit.
Based on interview & record review, the licensee did not comply with the section cited above. Mrs. Berthiaume stated that she checks on napping infants every 15 minutes, however she does not maintain logs. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Mrs. Berthiaume a sample copy of a 15-minute nap log today. She stated that she will start using the forms effective immediately. Licensee shall submit sample copies of infant nap logs (for 3 infants) to the department no later than 5/19/23 via email to - nancy.diaz@dss.ca.gov
