LPA met with Assistant Site Supervisor, Monica Marquez. There were 0 children present at the time of inspection. Hours of Operation are Monday - Friday, 7:30 am - 4:00 PM. LPA interviewed Assistant Site Supervisor and examined and photographed the water source (spigot) deemed at Action Level Exceedance. Spigots reported with 5.5 ppb or greater lead exceedance levels are as follows: Kitchen spigot F (not for children use) 25.75 ppb Assistant Site Supervisor stated that spigot has already been replaced and provided re-testing results. Assistant Site Supervisor stated that spigot is never used in any way for child care. All spigots inside classrooms are not in exceedance. See LIC809D for Type B deficiency cited. Exit interview conducted. Appeal rights were discussed and given to Assistant Site Supervisor on this date. Notice of Site Visit was provided and must be posted for 30 days.
