Based on observation, the licensee did not comply with the section cited above. LPA observed that children have access to the telephone cable located in the play area that was not secured properly. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: CORRECTED IMMEDIATELY. LPA observed that Mrs. Bautista removed the telephone cable and made it inaccessible to children.
Based on observation, the licensee did not comply with the section cited above. The pool gate was not latching properly. There is no direct access to the pool from the backyard. The pool gate is located next to the front door (on the side of the house). This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Bautista stated that she will have a repairman come today to fix the pool gate to ensure that it latches properly.
Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Bautista has not conducted a fire drill since she was licensed in 2022. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Bautista stated that she will conduct a fire drill with the children no later than 9/7/2023. She will submit a copy of the fire drill log to the department via email to: nancy.diaz@dss.ca.gov no later than 9/7/23.
Based on interview and record review, the licensee did not comply with the section cited above. Licensee's helper, Irma Ramirez Lopez has not completed the required Mandated Reporter Training. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Bautista stated that she will have her helper, Irma Ramirez Lopez complete the required Mandated Reporter Trng. (in Spanish) no later than 9/7/23. She will submit a copy of Irma's certificate to the department via email to nancy.diaz@dss.ca.gov no later than 9/7/23.
Based on record review, the licensee did not comply with the section cited above. Licensee failed to maintain immunization records on blue cards. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Bautista will obtain immunization record and update blue cards no later than 9/7/2023.
