Based on interview and record review, the licensee did not comply with the section cited above. Helper Neha Sharma and parents Govindbhai Parekh and Chandrikaben Parekh did not have fingerprint clearance on file. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Mrs. Battiwala stated that Neha Sharma will no longer provide care. She will obtain Livescan fingerprint clearance for her parents Govindbhai Parekh & Chandrikaben Parekh no later than end of business day of 9/6/2024.
Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Battiwala was unable to locate her current Mandated Reporter Training certificate. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Battiwala stated that she will complete the class online and submit a copy of the certificate to the department no later than 9/12/2024.
Based on observation, the licensee did not comply with the section cited above. LPAs observed 10 children present in the home supervised by the licensee without a helper. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Observed corrected today. Two children were picked up by parents at 1:41pm, leaving 8 children with licensee. Mrs. Battiwala stated that her husband will take time off from work to assist her with daycare effective 9/6/24. She will also obtain immunization for her mother Chandrikaben Parekh to be a helper. Mrs. Battiwala is aware that fingerprint clearance must be obtained prior to allowing Mrs. Parekh to work as a helper.
