Resolution: Licensee says she will obtained signed and dated copies of all state-required children's forms from the child's parents by due date of 10/4/2023. Licensee will email copies to LPA Poindxexter.
Based on record review, the licensee did not comply with the section cited above, as she did not possess and maintain the safe sleep logs for two of four infants in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will obtain or create the required safe sleep logs for both infants and keep updated as required. She will email copies of maintained sleep logs to LPA Poindexter by 10/4/23.
