Based on observation and interview, the licensee did not comply with the section cited above in not placing C1 in a crib or play yard as soon as possible, which poses an immediate health, safety or personal rights risk to persons in care. C1 sleep log began at 12:50 PM and LPA observed sleeping infant on a rocking chair with Staff #1 rocking him on or about 2:10 PM.

Resolution: Staff #2 removed C1 from rocking chair and C1 woke up. Licensee states she will speak to Staff regarding safe sleep regulations and will submit a written document of her understanding of this regulation and her plan of correction by email by 5/19/25.
