Resolution: Licensee stated she will send the department a copy of a current mandated reporter training certificate by email by 9/30/25. Licensee stated she will put it on calendar to ensure this training is taken once every 2 years.
Based on record review, the licensee did not comply with the section cited above in not conducting and documenting safe sleep checks and logs, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit to the department copies of safe sleep log for infants by email by 9/19/25. Licensee stated she will submit to the department a written statement of her understanding of this regulation and how she will ensure this violation is not repeated.
