Based on interview, record review, the licensee did not comply with the section cited above in by not having children's school enrollment verification for 6 out of 6 children enrolled which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will request school enrollment documentation from the children's parents to add to the children's file. Licensee stated she can complete the records and submit proof of completion to the Department no later than 11/13/2025.
Based on observations, interview of the licensee and record reviews, the licensee did not comply with the section cited above in their current Mandated Reporter Training completion certificate expired in December 2021, which poses as a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the online Mandated Reporter Training course. Licensee states that they will complete this training and provide LPA with the new training completion course certificate no later than 10/23/2023.
