Based on observation, and licenssee interviewthe licensee did not comply with the section cited above by means of not having a crib or play yard for infants to sleep in posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee brought out the play yard children will begin using as of tomorrow and stated she had two for children 0-12 months to use.

Based on record review the licensee did not comply with the section cited above by means of missing documentation for infants files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will have documentation in file once completed.

Based on LPAobservation, and licensee interview, the licensee did not comply with the section cited above by means of not having a safe place for infants to sleep in which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee pulled out a play yard for infants to begin sleeping in as of tomorrow.

Based on LPA observation and licensee interview the licensee did not comply with the section cited above by means of latches in the kitchen being broken and accessible, and other dangerous items in the backyard that are accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensse states she would fix it and send picture to LPA email.

Based on LPA observation, the licensee did not comply with the section cited above by means of not have a play yard or crib for infants in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would have this available for infants as of tomorrow.

Based on LPA observation, licensee interview and record review, the licensee did not comply with the section cited above b y means of being over capacity/ ratio which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will be speaking with the parent of one child to meet ratio.

Based on observation, interview and record review , the licensee did not comply with the section cited above due to having 9 preschool children, 1 school age and 4 infants which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would be speaking with a parent to cancel services and meet ratios.

Based on interview and record review, the licensee did not comply with the section cited above by means of not having sleep logs for LPA to review which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation, licensee interview and record review, the licensee did not comply with the section cited above by means of not having a certificate for the assistants in her facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee is going to have her assistant and husband take the training.

Based on record review, the licensee did not comply with the section cited above by means of not having this document in file for 1 of 1 children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will be making copies and give it to the parents to complete.
