.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 5.0 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. Based on observation, interview, record review, the licensee did not comply with the section cited above, child #1 missing sign out signatures for 09/09 -09/12, 09/16-09/20, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited. Director (S3) listed on the Licensing Information System - Facility Profile is no longer employed at the Preschool and the new director was not reported to CCLD which poses/posed a potential health, safety or personal rights risk to persons in care.
