Based on record review, the licensee did not comply with the section cited above, staff 1 Mandated Reporter Training (AB1207) was completed on 01/19/2022 and there is not a current Mandated Reporter Training (AB1207) certificate on file, staff 2 does not have a Mandated Reporter Training (AB1207) certificate available for review, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees tohave staff 1 and staff 2 complete the Mandated Reporter Training (AB1207). Licensee agrees to provide proof of staff 1 and staff 2 Mandated Reporter Training (AB1207) certificate of completion to Community Care Licensing by the end of the business day on the POC due date of 05/28/2025. www.mandatedreporterca.com

Resolution: Licensee agrees to obtain proof of staff 1 and staff 2 required immunizations and maintain proof at the facility. Licensee agrees to provide proof of staff 1 and staff 2 immunizations (MMR, Pertussis, Influenza or Influenza Declination) to Community Care Licensing by the end of the business day on the POC due date of 05/28/2025.

Based on record review, the licensee did not comply with the section cited above. The Child Care Facility Roster (LIC9040) is incomplete during this visit. With the licensees own admission, there are 7 children enrolled and the LIC9040 list 5 children enrolled. The 3 children observed in care at the time of visit are not listed on the LIC9040, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and maintain the Child Care Facility Roster with currently enrolled children. Licensee agrees to provide a copy of the Child Care Facility Roster to Community Care Licensing by the end of the business day on the POC due date of 05/28/2025.
