Based on record review, the licensee did not comply with the section cited above, S1 Mandated Reporter Child Care Providers Training (AB1207) was completed on 11/10/2022 and there is not a current Mandated Reporter Child Care Providers Training (AB1207) on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to provide proof of S1 Mandated Reporter Child Care Providers Training (AB1207) certificate of completion to Community Care Licensing by the end of the business day on the POC due date of 09/08/2025. www.mandatedreporterca.com

Based on record review, the licensee did not comply with the section cited above. Staff 1 (S1) and Staff 2 (S2) Pediatric CPR/First Aid Training was completed on 02/08/2024 (not an approved EMSA vendor and online) and there is not a current Pediatric CPR/First Aid certificate on file with an approved EMSA vendor, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1 and S2 non-expired Pediatric CPR/First Aid Training with American Heart Association, American Red Cross, or Emergency Medical Services Authority (EMSA) approved vendor and provide proof to Community Care Licensing by the end of the business day on the POC due date of 09/08/2025. www.emsa.ca.gov or Childcare Provider | EMSA https://emsa.ca.gov/Childcare_Provider/

Based on record review, the licensee did not comply with the section cited above. The Child Care Facility Roster (LIC9040) is incomplete during this visit, with the licensees own admission, there are 13 children enrolled and the Child Care Facility Roster list 18 children enrolled, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete and maintain the Child Care Facility Roster with currently enrolled children. Licensee agrees to provide a copy of the Child Care Facility Roster to Community Care Licensing by the end of the business day on the POC due date of 09/08/2025.

Based on record review, the licensee did not comply with the section cited above. Child 1 (C1) does not have an Infant Sleep Log that documents the time of each 15-minute check at the time of the visit. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document and maintain an Infant Sleep Log for C1 that documents the date, infant’s name, and time of each 15-minute check. Licensee agrees to provide proof of the 15-minute checks for C1, to Community Care Licensing by the end of the business day on the POC due date of 09/08/2025.

Based on observation, the licensee did not comply with the section cited above in that facility had hazardous items accessible to children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Hazardous items removed, area made inacessible during visit.

Based on record review, the licensee did not comply with the section cited above in that 2 out of 2 persons had expired mandated reporter which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit mandated reporter renewals by POC due date 11/11/2022 for licensee and employee.
