Based on record review, the facility did not comply with the section cited above in staff #1 not proving proof of TDAP and MMR immunizations which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in two of two staff files. At the time of this inspection a staff person was not present who had current training in pediatric CPR/First aid which meet the requirements of Health and Safety Code Section 1596.866. On line training does not meet the requirements of Health and Safety Code Section 1596.866. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the information received, outlet (A) had an ALE of 26.0000 and outlet (D) had an ALE of 8.9000, which is out of compliance with the allowable ALE of 5.5 ppb. Outlet A was utilized 8/2022 until 2/2023. This poses a potential health and safety risk for the children in care.
Based on record review and interviews, C1 wandered away from the classroom and was without the supervision of a teacher for approximately 35 minutes. This posed an immediate risk to the health and safety of children in care.
(f) At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to Health and Safety Code Section 1596.866 shall be present when children are at the child care center or offsite for center activities.

Based on interviews and record review, S2 was alone on the playground with six children on April 21, 2023, therefore S1 was not have provided direct supervision of S2. This poses a potential Health and Safety risk to persons in care.
