Based on observation, interview and record review, the licensee did not comply with the section cited above, Licensee's Mandated Reporter Certificate has expired, Staff's Mandated Reporter Certificate was unavailable for review, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Staff agree to complete and/or renew Mandated Reporter Certificates and submit proof to LPA on or before the listed POC date of 07/19/2024. Submission can be sent via email. justin.giese@dss.ca.gov

Based on observation and record review, the licensee did not comply with the section cited above, LPA was unable to verify Staff's immunizations records, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in one out of five children's records reviewed was missing immunization records, which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA’s observation and record review the licensee did not comply with the section cited above. Licensee, Staff 2 and Staff 3 have not completed Mandated Reporter Certification, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands the importance of this regulation and will have staff complete Mandated Reporter Certification. Completion of missing documents will be submitted to LPA on or before proof of correction date listed above.

Based on LPA’s observation and record review the licensee did not comply with the section cited above. Licensee did not have a facility roster to review, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands the importance of this regulation and will review children's files and complete a facility roster. Completion of missing documents will be submitted to LPA on or before proof of correction date listed above.

Based on LPA’s observation and record review the licensee did not comply with the section cited above. Licensee and Staff 3 did not have current CPR certification available to review, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands the importance of this regulation and will locate or have staff complete CPR certification. Completion of missing documents will be submitted to LPA on or before proof of correction date listed above.

Based on LPA’s observation and record review the licensee did not comply with the section cited above. Licensee and Staff 2 did not have proof of Immunizations for MMR and Tdap availble to review, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands the importance of this regulation and will have staff locate/complete necesary documents ahowing proof of immunizations against MMR and Tdap. Completion of missing documents will be submitted to LPA on or before proof of correction date listed above.
