Resolution: Licensee agrees to have S2 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to provide proof of S2’s MMR, Pertussis, Tuberculosis, and Influenza required immunization's to Community Care Licensing by the end of the business day on the POC due date of 10/07/2025.

Based on record review, the licensee did not comply with the section cited above. Child 4 (C4) and 5 (C5) Consent for Emergency Medical Treatment (LIC627), is not available for review during this visit, which poses a potential health, safety or personal rights risk to persons in care.

Licensee agrees to provide proof of C4’s and C4's Consent for Emergency Medical Treatment (LIC627) to Community Care Licensing by the end of the business day on the POC due date of 10/072025.
