Based on record review, the licensee did not comply with the section cited above, staff 1 Mandated Reporter Child Care Providers Training (AB1207) was completed on 01/16/2023 and there is not a current Mandated Reporter Child Care Providers Training (AB1207) on file, staff 2 does not have a Mandated Reporter Child Care Providers Training (AB1207) certificate available, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to staff 1 and staff 2 complete] the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to provide proof of staff 1 and staff 2 Mandated Reporter Child Care Providers Training (AB1207) certificate of completion to Community Care Licensing by the end of the business day on the POC due date of 07/01/2025.

Based on record review, the licensee did not comply with the section cited above. Child 5 (C5) proof of required immunization's are not on file, nor available for review during this visit, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain and maintain proof of C5’s required immunization's. Licensee agrees to provide proof of C1’s required immunization's to Community Care Licensing by the end of the business day on the POC due date of 07/01/2025.

Based on record review, the licensee did not comply with the section cited above. Child 5 (C5) Affidavit Regarding Liability Insurance (LIC282) is not available for review during this visit, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C5' s authorized representative complete form LIC282 and maintain proof at the facility. Licensee agrees to provide proof of C1’s Affidavit Regarding Liability Insurance LIC282 to Community Care Licensing by the end of the business day on the POC due date of 07/01/2025.

Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee stated they did not have Mandated Reporter Certificates for them self and Assistant for LPA to review, this poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant agree to complete General and Provider Mandated Reporter Training and mail or email completed certificates to LPA on or before the above listed POC date of 02/09/2023.

Based on observation, and record review, the licensee did not comply with the section cited above. While conducting Licensee and Staff record reviews, LPA observed both Licensee and Staff CPR/First Aide cards to be expired, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands the importance of this regulation and stated they and staff enroll in and complete CPR/First Aide certification renewal on or before the above listed POC date of 02/09/2023.
