Based on record review, the licensee did not comply with the section cited above. Child 5 (C5) does not have an Infant Sleep Log that documents the time of each 15-minute check at the time of the visit. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document and maintain an Infant Sleep Log for C5 that documents the date, infant’s name, and time of each 15-minute check. Licensee agrees to provide proof of the 15-minute checks for C5, to Community Care Licensing by the end of the business day on the POC due date of 09/25/2025.

Based on record review, the licensee did not comply with the section cited above. Child 1, 2, 3, 4, and 5 (C1, C2, C3, C4, C5) does not have a copy of the Parent Notification Additional Children In Care (LIC9150) at the time of the visit. By the licensee own admission, there are at least 8 school age children enrolled at the facility and 16 children enrolled. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide a copy of the completed Parent Notification Additional Children In Care (LIC9150) for C1, C2, C3, C4, and C5 to Community Care Licensing by the end of the business day on the POC due date of 09/25/2025.
