Resolution: Per licensee, staff will provide documentation of the required immunization by 3/26/2025. Licensee will provide documentation to the Department.

Based on interview and record review, the licensee did not comply with the section cited above in one of three children (C2) present did not have a child file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, the child will not be accepted into care until all required documentation is obtained from the authorized representative. If C2 is accepted into care a complete file will be created. Documentation the file has been created will be submitted to the Department.

Based on observation, the licensee did not comply with the section cited above when Child 1 was observed sleeping in a bassinet. A bassinet is not an approved sleeping device for infant safe sleep, this poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, the bassinet will no longer be used as a sleeping device. Only playyards or cribs will be utilized for the infants. The bassinet removed form the on limits space.

Resolution: Per Licensee, the fire extinguisher will be serviced. Documentation will be submitted to the Department verifying the service was completed.

Based on observation and interview, the licensee did not comply with the section cited above when Child 1 was observed sleeping on top of a folded blanket in the bassinet, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, blankets will not be added to the play yard or crib. The blanket was removed. A video regarding safe sleep will be sent to the Licensee, the Licensee will submit documentation to the Department confirming the video was watched.

Based on observation, the licensee did not comply with the section cited above when LPA observed a child wrapped in a blanket and falling asleep in a play yard. LPA observed a pacifier in the child's mouth with a pacifier clip attachment. LPA also observed a loose activity mat and loose toys in the play yard with the infant. This observation poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: All items were removed from the play yard during the inspection. Licensee will review the safe sleep regulations and videos. Licensee will send LPA written documentation that the safe sleep training has been completed
