Based on observation, the licensee did not comply with the section cited above. Licensee was found with 10 children in care and no assistant was present, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee requested a parent to pick up 3 children, and remained with 7 children only. Licensee was instructed on AB633 requirements, and was provided with the forms to obtain parents acknowledgements. Licensee shall submit the completed forms Lic9224 to licensing program no later than June 27, 2024

Based on observation the licensee did not comply with the section cited above. Licensee has a construction in the back of the home that used to be a playground for the children in care, and is now a separate living unit. This separate unit has its own address number and its own entryway, this poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit an updated sketch showing the current lay out of her FCCH no later than 7/05/24

Based on observation the licensee did not comply with the section cited above. Licensee was found providing care to seven children: 3 infants, 4 preschool, no school age present. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee called parent of the siblings children #1, and #2 to pick them up during the inspection. Licensee will submit an statement that she understands the ratio for an small licensee by the close of business of 4/28/23. LPA discussed the requirements of AB633 to licensee and provided her the AB633 fact sheet and a copy of Acknowledgement of Receipt of Licensing Reports (LIC 9224) and licensee understands the requirements. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Licensee will submit a copy of the signed forms lic9224 on or before May 5, 2023.

Based on record review the licensee did not comply with the section cited above. Licensee does not document in a log every 15 minutes when infants are asleep.which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall start documenting the infants sleep every 15 minutes. A log sample was provided to licensee. Licensee shall start documenting the infants sleep starting 4/27/23 and continue until children reach the 24 months old. Licensee will submit a copy of the log by 5/11/23, the log shall include the period 4/27/23 to 5/10/23.
