Based on observation, the licensee did not comply with the section cited above LPA observed a un-serviced 2A10BC fire extinguisher last service on 11/06/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall take the fire extinguisher to get service and submit a picture of the service tag to CCL by POC due date.
Based on observation, the licensee did not comply with the section cited above LPA observed hazardous items (dolly, workout items & weights) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit pictures of hazardous items removed and an updated LIC 999 once backyard is within limits. Licensee provided LPA an LIC 999 that states backyard is off limits to children in care during visit.
Based on record review, the licensee did not comply with the section cited above LPA observed child (C-1) did not have a every 15 minute logs on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit proof of every 15 minute checks for child (C-1) to CCL by POC due date.
