Based on record review, the licensee did not comply with the section cited above, Licensee has not documented the check up on infant at sleep time, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall start documenting the check ups on infants sleeping and untill infants reach the age of two years old. Licensee will submit a copy of the log (provided to her) on 6/26/24 or 6/27/24 with all the check ups documented for the period 6/12/24 to 6/26/24.
Based on record review the licensee did not comply with the section cited above, Licensee's helper has expired Mandated Reporter training since 10/11/23, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit to Licensing Program a copy of the training certificate for her helper Michelle no later than 6/27/24.
